I used to have monolids but they turned double as I got older. The crease is heavy and uncomfortable. When I tilt my head up the crease disappears and the heaviness diseappears. However, I want to preserve the crease but also get rid of the heavy feeling. Is there a procedure for this?

Although an in-person exam would be necessary to provide a definitive answer, based on the photo you have provided it does appear you would be a suitable candidate for upper blepharoplasty to create a more defined crease. During the surgery a small amount of fat and skin will be removed. If you want the best idea of what is possible for your eyes, do your due diligence, find an experienced surgeon, and have him/her evaluate you in person and discuss your options. #drdonyoo

It is not rare to hear this from patients. The anatomy of the eyelid skin, fat, and levator can change over time. A very conservative DES (likely no fat removal and little to no skin removal) would prevent the skin just over your lashes from hanging over and causing the heavy feeling.
