Does SARPE have to be the primary operation or can SARPE and LeFort be performed during the same operation? (Photo)

Question

I'm 29. I've class 3 and a bit narrow upper jaw. But I don’t suffer from any snoring or apnea. Doctor thought SARPE is not necessary. I use Damon braces. He said they could help that too. But I've aesthetic concerns about narrowness. To me SARPE and LeFort I are nearly the same things. So I wonder if palatal expansion can be performed during the maxillary advancement operation. Like not in couples weeks by expander but at once by separating the palate and screwing them in final position.

Lefort 1 Multi Piece Osteotomy is a surgical procedure used to correct skeletal malocclusion, dentofacial asymmetries, and midface deficiencies or excesses by readjusting the position of the carrier segments of the teeth. I believe that a more detailed examination is necessary to decide on the treatment plan. It's possible to create a more precise treatment plan with a tomography, intraoral scanner, and professionally taken photographs.

In this case, upper jaw is cut into 3 pieces, widened and brought forward.   it is called segmental osteotomy
