I have a Class I relationship after braces, but I will need MMA surgery to open up my airway (movements will be about 10mm each). Since the jaw comes forward and down when it is advanced, is there any way to prevent the face elongating during this surgery? I don't like my profile, but I like my face from the front and if I jut my jaw forward I look like I have acromegaly. I don't have a gummy smile, so I can't do anterior impaction as far as I know.

You can have a combination of an 'upper jaw' surgery to raise the maxilla/upper jaw and reduce the gummy smile, if present. Cheek implants at the same procedure can then improve the cheekbone zone. Not uncommon and you can maintain the same bite.
