February 11, 2014 I had breast augmentation. Pre-op, I was a 32 AA. Received Mentor 325cc high profile silicone implants taking me to a 32D. I absolutely hate the size, but my surgeon will not consider revision until 1 year post-op. At one year post-op, should I be concerned with my skin and tissue being stretched? He said downsizing (to 250cc) would cause loose skin and droopy breasts. I don't know if this is his honest opinion or he's just trying to discourage me from any revision. Thank you!

How much depends on your skin and breast evaluation in a physical exam. If you are high profile now, is it also a profile concern that you have? Perhaps a moderate or moderate plus profile would achieve what you're looking for without too much sagging with an appropriate volume change. Ask your plastic surgeon. Good luck

Answer: Concern over droopiness with smaller implants I am sorry you do not like the size of implants. Size and the way you look in clothes is a very personal decision and being self conscious about your implants defeats the purpose of having them. You certainly look fairly full i the picture you submitted but it is difficult to tell how much loose skin you have. If you do not have a lot, going down in size should not cause too much laxity of the skin envelope. I recommend an assesment from a board certified plastic surgeon to dtermine what your options would be.

Answer: Sagging with downsizing of breast implants You are young and your breast and skin tissues should retract after downsizing your breast implants. Extreme downsizing can certainly cause sagging. However going from 325 cc down to 250 cc is, in my opinion, unlikely to cause significant sagging in a young person with good skin tone.However I do share your concern that waiting a year will give the implants a chance to stretch your breasts more than is occurring in the first 3-6 months.It is your right to have the breast implants that you want. If he still do not want these implants at 6 months, if you're my patients I would remove them.

What is it about your current look that you find concerning? Knowing that would help in determining just what would be done. Before you have this done, ask for the dimensions of her current and planned implants so you can better visualize just how much (or little) of a difference it will make. You may find with the extra time and settling, that you may be okay with your current look. But in the end, your surgeon does want you to have the best result possible and to be as happy as possible... because if he wasn't, he wouldn't last very long in the competitive environments we work in.
