Trimming a long graft - can this be done without a full revision? (Photo)

Question

I had a revision rhinoplasty overseas last year and the collumellar strut is too long and presses into my lips when I smile or laugh and it causes discomfort and an ugly crease beneath my nose. Can the graft be shortened without a full revision?

Strut crafts for over 25 years, and occasionally they can click on underlying bone of the premaxilla. That is the case, we will shorten the graft through an incision in the mouth between the upper lip and gum. This usually leads to an immediate Improvement, and there is typically no downtime. Hello and thank you for your question. Based on your photograph, the excessive columellar length can be corrected with a revision. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.

It's likely we can do that in the office. What is the graft? Is it cartilage? Even more likely.
FYI, it certainly could be that the crease is not related to the graft...
