What is the most economical way to improve the sagging corners of my mouth as well as smooth the wrinkles in my forehead? Since I am 44 years old, is it better to get some type of facelift for a more permanent solution or would paying for fillers and Botox every several months be more cost effective? Thank you.
Answer: Facelift Eliminates Sagging The only real “cure” for a sagging face is a facelift, which will set your clock back around 10 to 15 years – a dramatic difference. Fillers can restore volume, but the lift they offer is minimal. Plus, they must be re-administered frequently, which quickly adds up to the cost of surgery over time.From your photo, you seem to have some sun damage in addition to sagging. The best course would probably be laser resurfacing to even out your skin tone and smooth some of the wrinkles, plus a facelift with fat grafting to get rid of the sagging and restore volume to your midface. You might still need intermittent Botox to control the glabellar lines (11s). I hope this helps.

The recovery & downtime is very minimal. There are usually no drains, most people can go out the same day, you can shower the next day, there're no scars behind the ears and you can wear your hair up anytime and most patients take no pain medicine. The result is usually the same in selected patients as a full scar facelift but it really depends on your exam. Surgery is not for everyone. In today's day and age, there are many nonsurgical options to consider although surgery usually gets the most improvement specifically of the neck and jowls. I would recommend that you see a board-certified plastic surgeon in your area that is a member of both ASPS & ASAPS and performs a lot of mini facelifts and neck lifts but also offers all the nonsurgical options such as injectables and laser treatments. Go over your concerns with them and make sure above all, you walk out of there with several options and an idea of the pros and cons to each of them as well as realistic expectations. After several years, you have in effect paid for a surgical procedure. Around the corners of the mouth, non-surgical correction is best done with fillers that also generally last no more than 6 months. The average well-done lower face lift (not a mini lift) should last on average close to 10 years and will also address the jowls. This is a highly individual decision that really can only be fully evaluated during an in-person consultation. At age 44, many people will try to hold off on having surgery by using BOTOX and fillers for a few years, but surgery does give a better long-term solution for the mouth. It is hard to talk economics without a one-on-one consultation.

In my book entitled, “The Fine Art of Looking Younger”, I mentioned there that facial aging has two processes: one is volume loss, where we lose bone, muscle and fat and the other one is sagging. These have been seen in all radiologic studies. When you look at a much older person, their face looks a little smaller. This is why even in the Upper East Side that no matter how much money people make, they can’t be lifted, pulled and plumped enough to make them look like they’re 20 years old even though they’re 70.
In my evaluation, the first thing I assess is if there is any slide of the skin where I am able to lift. Very commonly, women in their 40’s will do something called a mirror lift. This where they hold their face and push it forward to see if the cheeks go higher, the jawline improves or if the neck looks a lot better. Loose skin along with the supporting tissue called SMAS contribute to the advancement of sagging skin.
If the skin is only slightly loose or it has little facial tone, I don’t advocate a lifting procedure. However, if there is a lot of looseness, I tell the patient to consider a facelift first because to me restoring structure or adding volume makes the most sense. If you’re in a situation where you don’t have that much to lift, or you don’t have that much time, then injectables such as Botox and Dysport for the dynamic lines such as the frown lines, the crow’s feet lines or even for the lid lifting effect make a lot of sense. We use fillers such as Radiesse or sSulptra and even use fat to help the volume loss with bone, muscle and fat which are the deeper tissues. For the dermal or the backbone of the skin, we’ll use a hyaluronic acid such as Restylane and we also use platelet-rich plasma which is derived from your own blood to help stimulate the quality of the skin. It stimulates blood flow and collagen so that the skin looks healthier.
The goal is looking your best in whatever age you’re at and always looking youthful. If you are someone with a lot going on, whether it’s in a career or in the social world, having a beauty doctor gives you an edge. A doctor that you can connect with and take care of you is really important. One set of treatments that are non-invasive that you have to repeat does not mean that doing one surgical procedure eliminates the need for those other non-invasive procedures. It’s the balance of restoring position and structure as well as restoring volume and reducing the lines and wrinkles so that you look younger.
