Why are my eyes small and round after a lower transconjunctival blepharoplasty? (Photos)

Hello Lamb and Lion,Your round eyes, although not that significant, could be caused by swelling or surgical positioning of the angles of your eyelid. Hard to tell without an examine. Yes it is resolvable. It can be corrected if it doesn’t completely improve 90 days after surgery. A simple surgery can correct it. This is one of the common problems that can occur from lower eyelid surgery. Based on m6 experience the outcome is good. Most of the healing of lower eyelid surgery can take 90 days so have patience. My suggestion is to have this discussion with your surgeon or an expert Oculoplastic surgeon, Hope this helps. Cheers 

I honestly don't see the smaller part based on the photos. Usually most lower bleph issues resolve but in case they don't there are very simple ways to change the contour of the lower lid. If you get to that decision making point, then show the surgeon with your finger how you would prefer the contour to be. Changing the sag in the lateral portion of the lid may improve on thing while actually making the eye opening a little smaller.

Thank you for your photos. It appears that you have slight rounding of the lower eyelids after your transconjunctival (TC) blepharoplasty during your healing period. The advantage to having a TC bleph vs an external approach is that the muscle and skin are not violated. This leaves more support to keep the eyelid in the correct position. That being said, you may have residual tissue swelling, inflammation, or weakness of the eyelid muscle which is all expected and normal during the healing process. I usually have patients begin eye blinking exercises and taping to help tighten the eyelids back to the original position as they heal. For patients who's eyelids are not fully restored once healed, a separate lid tightening procedure (canthopexy) can be performed. Hope this helps!
