How to get rid of deep self-harm scars and find an understanding doctor experienced in this?

I have hundreds of self- harm scars on my arms. All are now faded to white, thin but deep, leaving my skin quite textured. I have tried creams, silicone sheets, and microneedling, and scar revision surgery on a few thicker ones which helped. But they still look obviously self inflicted. I have gone to a plastic surgeon for a consultation but he just became very uncomfortable. What can do for these that is proven to work? How do I find a doctor who has experience with this type of scarring?
