Tightening the lateral canthus helps make the palpebral tissue (space between the two eyelids) look more almond and less round. However, it can also uncover latent ptosis. Very often when I am correcting Post Upper Blepharoplasty Syndrome (PUBS), which includes upper eyelid ptosis, high or absent upper eyelid crease, hollow upper eyelid sulcus, compensatory eyebrow elevation, crepey eyelid platform skin and lash ptosis caused by prior blepharoplasty, the first thing I do surgically is repair the disinserted lateral canthal angle with a canthoplasty. This does bring out more ptosis but that is being corrected with the reconstructive surgery to address PUBS. Correcting a little more ptosis is not an issue and this helps restore the almond shape to the eye.
