My zygomatic bone's give me an odd and rather undesirable appearance which I do not like. Also, I've heard that cheek bones which are frontally flat can be problematic for zygomatic bone reduction surgery. Any suggestions as to which method of zygomatic bone reduction surgery would best suit my facial structure, If surgery is possible.
Answer: Wide Cheekbones Thank you for your pictures. You have a very long face but a slightly wide upper face due to your zygomatic bone/cheekbones. I think a strategy would be to make an incision inside the upper lip and slightly contour the width of your cheekbones this should be a subtle and not aggressive approach.

Answer: Zygomatic Bone Reduction This would need a surgical approach and honestly I feel would make you look less masculine. I suggest a formal facial sculpting evaluation. Best, Dr. Emer.

Answer: Zygomatic Reduction It appears that your area of zygomatic concern is its main body which is very prominent in your lean face. Treating this zygomatic prominence is different than zygomatic arch reduction which narrows the facial width behind it. Treatment options include an intraoral burring method or an anterior sagittal zygomatic osteotomy with wedge removal and narrowing. Because more soft tissues attachments are preserved, the latter is preferred.

Answer: I've heard that cheek bones which are frontally flat can be problematic for zygomatic bone reduction surgery. Hi, I have performed many facial shaping and contouring procedures for over 30 years using dermal fillers or facial implants (chin, cheeks). From the photos, your cheeks are flat anteriorly (in the front) with high points on the lateral aspect of the malar bone, not the zygomatic arch. Collapsing the zygomatic arch creates an in fracture of the arch in an attempt to narrow the face. This isn't a reasonable or effective approach to shape the face IMHO. I'd also like to point out that your chin and mandibular angles are also weak. A weak chin creates an imbalance making the nose appear larger, the mid face top heavy and the lower face look short that de-emphasizes the lips and allows early formation of a double chin. Chin augmentation using a chin implant will add projection to the chin creating harmony and balance to the lower face. I have found that placement of a silastic chin implant, through a small curved incision under the chin (also allows excess skin removal) to be very safe, quick and highly effective. Hi, based upon the beauty principles outlined in my book on face and body beauty men look ruggedly handsome with angularity in the cheeks, chin and mandibular angles. From the photos, your cheeks are flat anteriorly (in the front), the chin is weak as are the mandibular angles. This combines to create an elongated oval shaped face. The following procedures would create a more ruggedly handsome face.Cheek augmentation with cheek implants to add angularity to the cheeks and mid face. This would be combined with rasping of the lateral malar bone to reduce the high points and further define the cheek shape.Chin augmentation, using a chin implant, to add projection to the chin creating harmony and balance to the lower face.Augmentation of the mandibular angles using a dermal filler (jaw implants have too many unwanted side effects IMHO) to add volume, angularity and flare to the jaw line.Hope this helps.
