First let me congratulate you on both quitting smoking and initiating a healthy diet. These will hold you in good stead for life. Your px suggest significant loss of volume in the submalar region of your cheeks. Happily, these days we have wonderful volumizing fillers that can reflate this region and which typically engender a "Wow!" response immediately following treatment. These include Voluma XC and Radiesse Plus, which I would use in my Upper East Side Manhattan office and Stylage XXL and Emervel Volume, which I would opt for in my Israel satellite facility, where a far greater number of regulatory agency approved injectable volumizers are available. Results typically last about 1.5-2years. Personally, I would not recommend fat injections, a procedure I gave up performing over twenty years ago, as this relatively expensive technique involves a double minimally invasive procedure of removing the fat from the abdomen, buttocks, etc, and then reinjecting with large bore cannulas. Nor would I recommend the use of Sculptra, which I gave up using several years ago. This, too, is relatively expensive, does not give immediate results, and typically requires about two to three treatment sessions spaced over a several week intervals to achieve optimal results, which last about two year..Make sure to consult with a board certified aesthetic physician with experience and expertise in nonsurgically treating the male face. I would say you have two main options to help improve the loss of volume in your cheeks. The first option would be surgically placed implants. The second option would be using fat transfer or an injectable filler. You should be able to get improvement either way, but there are advantages and disadvantages of each method. I would consult with a facial plastic surgeon or plastic surgeon regarding the implants and a facial plastic surgeon, plastic surgeon or dermatologist in regards to any possible fillers or fat transfer.
Fillers placed in the cheeks offer a temporary augmentation when there is a flat cheek profile. Silastic cheek implants offer a permanent augmentation to the cheeks. Cheek implants are manufactured in a large variety of shapes and sizes, which is best determined at time of examination and consultation. Fat injections are an option but the most reliable method would be specially designed cheek implants that can augment the submalar and upper maxillary area where your cheek volume loss is located.
Fillers would help you. The trouble is that the amount of improvement would minimal without using large amounts of product, I would guess at least 5cc per side. 5 cc is one teaspoon. Fat is a reasonable alternative, assuming that you have any. In my practice it is cheaper than 100 of injectable filler, though it is not as reliable.
