I had a breast reduction in February 2020. I went from a 38 DDD to a 38 D. I was told it would be a simple procedure with an incision around the areola, one down the front of my breast and one across the bottom. My doctor mentioned I would have a slight indentation at the bottom which I figured was normal with a anchor lift but that's not what I got. I don't know what he did or why he chose the procedure he did but it has left me with horrible scarring, dog ears and VERY square breasts.

Yes it is possible to have a second breast reduction. Discuss your history of keloid formation with your surgeon. She/He will work with you to help promote favorable scars and can watch for early signs of keloids. I've included a video I hope you find helpful.

By definition, the nipple is taken off and sewn back on to the breast like a skin graft. It will not have nipple sensation once healed. It may gain some skin like sensation but even that is unlikely.
