Whether or not to change the appearance of your nose is a very personal decision. If you do decide to augment or refine your nose, this can be performed during Asian rhinoplasty using autologous grafts (your own tissue). Cartilage from either your septum and ears, or rib, may be used to augment your nose.

In looking at your pictures, your overall nasal length looks ok but your tip could be projected a bit more as is the case with most Asians who seek rhinoplasty in my practice. As others have mentioned this can be accomplished through stacked tip grafting. In addition your chin is rather weak and depending on your dental occlusion a variety of options are available to you to augment this area and provide better facial harmony. The rhinoplasty procedure can address raising the bridge of the nose with cartilage grafting techniques. When there is a recessive and weak chin profile present, a chin implant is suggested to give further balance and proportions to the profile of the face. Dear jc1127,Thank you for your clinical post and photograph. Analysis of your profile shows that thetakeoff of your nose is slightly low and weak and your chin slightly underprojected. You do have a long face withyour forehead and upper third and lower third being relativelyproportionate. The nose itself could belengthened slightly through the use of soft tissue fillers such as Juvederm orRestylane and this will give you a good idea as to the efficacy of using longerterm tactics and strategies such as cartilage grafting to the bridge. Your nasal bridge is not classicallydepressed or scooped out as it might be and only very subtle changes may beneeded in the radix or top of your nose and nasal tip. If you do make the nose slightly larger andlonger this will exaggerate the under projected chin and you may also considerinjecting Juvederm Voluma or other soft tissue fillers in the chin to assessthe impact of this nasofacial balance, aesthetics and proportions.Long-term strategies such as Genioplasty or bony chin augmentationtogether with cartilage grafting to the nose may be options for you. Your projection and nasal length can be increased with cartilage grafts. For the past 35 years I've avoided the use of synthetic implants because of the increased risk of movement, extrusion, and infection. Having said that, this can be done but I question the need for these changes. However, chin augmentation would significantly improve your profile.

If too much cartilage was removed, then your tip will look amorphous and round. Kenalog can help with the internal scar, but your main issue is the need to build a strong internal support structure of cartilage to get the tip back to a nice shape. I would most likely use some kenalog in...
