Dear jc1127, Thank you for your clinical post and photograph. Analysis of your profile shows that the takeoff of your nose is slightly low and weak and your chin slightly under projected. You do have a long face with your forehead and upper third and lower third being relatively proportionate. The nose itself could be lengthened slightly through the use of soft tissue fillers such as Juvederm or Restylane and this will give you a good idea as to the efficacy of using longer term tactics and strategies such as cartilage grafting to the bridge. Your nasal bridge is not classically depressed or scooped out as it might be and only very subtle changes may be needed in the radix or top of your nose and nasal tip. If you do make the nose slightly larger and longer this will exaggerate the under projected chin and you may also consider injecting Juvederm Voluma or other soft tissue fillers in the chin to assess the impact of this nasofacial balance, aesthetics and proportions. Long-term strategies such as Genioplasty or bony chin augmentation together with cartilage grafting to the nose may be options for you. Computer imaging using two dimensional and three dimensional changes can help the rhinoplasty surgeon to find the types of improvements that you may be seeking and give you a chance to assess your nasofacial balance and proportion before committing.
