This is one of the more interesting questions I have seen in a while! You will not find a reputable board certified plastic surgeon that would be willing to do this as it would be considered an unethical procedure. That being said, if you search hard enough, you will always be able to find someone willing to take your money that doesn't put your safety and best interests first, so please trust me when I say you should not try to have this procedure done. Biological tissues change during the healing phase. Unlike a heart that you cut out of paper, if you designed a heart shaped areola, it might look "heart-like" on the operating table, but as the scar heals, it will contract and will distort the shape. Even if creating heart shaped areolas was ethical for plastic surgeons, the long term result would just be a scarred, distorted areola. If you are determined to have heart shaped areolas, a highly skilled tattoo artist would be your best bet!
