I got a tip plasty 4 years ago and the swelling was hell. No interventions and the (top nyc) doctor said it was because of spongy skin that the results were less than ideal. Finding it had to trust anyone after this experience. Should I try to fix this? Very bulbous and misshapen.

Having said that, though, whether a revision can narrow your tip depends on how much tip cartilage is left, what its strength and shape and position are, and how much of the width of your tip is the cartilage vs the thick skin. For example, if the cartilages are strong and broad and controlling the tip width, then a revision very well might help. But if the cartilages are thin and weak, and all of the tip width is your skin, then you're probably done. If I had to guess, based on this photo, I'd say that more can be done. But as I said, I or anyone else would have to feel your nose first.
