What can I do if the surgeon didn't remove enough breast tissue for insurance?

I had a breast reduction done last week. I told my doctor's office that my insurance required 500g or 22% of body surface area. In surgery, only 314g per breast was removed. It puts me at 1.69 on the sliding scale. The chart on my insurance page puts me at needing about 360 grams removed per breast. What options do I have? Has anyone ever appealed after surgery? I would not have gotten the surgery if I knew we weren't going to meet the requirements.
