After the surgical procedure, is there a way to ensure that the dimple is only present when smiling and not when not smiling? Thanks!

Good questions!  (1) It may not matter much, but if you are older with wrinkled skin the dimple may not be as visible. Hi, yes it could be done one side dimple. It is a very easy procedure and I use anesthetics to the area, by this way there would be no pain.
