The area around the lips does not have any natural fat pad. There are several muscles including the orbicularis oris. The muscle appears to be prominent in the photo due to loss of soft tissue volume around it. The natural fat pad that is closest to the lips is the premental fat pad which rests along the lowest part of the chin and appears healthy in the photo. I recommend nanofat and possibly microfat for the depressions around the orbicularis oris. Nanofat is rich with stem cells and helps the tissue rejuvenate itself. Microfat adds volume. A filler could help, however it will be temporary (lasting around a year).
