You are seeing the malar fat pad, which sits on top of the bone there.  This can get more prominent as we age.  The fillers to the cheek can often help pull this fat pad up, or at least change the shadows so you don't see it as much.   You may want to try a little filler now below the fat pad in the demarcation line you have there.  If you don't like it, it can be removed.

For unique cases where a variety of modalities could be used to treat the condition, I would suggest an in-person or virtual consultation to develop a custom and effective treatment plan. One thing is to consider fat transfer. I do think volume (filler or fat) can help but people often need a significant amount and with fat we have essentially unlimited supply.It is worth researching at the least.Hope this helps.Best of luck,DrC
