That tiny lump seems to me a overly small piece of grafting that has moved due to not being stitch-secured, just placed as loose piece.

Your tip has not been worked (practically) at all, being a thick skinned case you need very strong and pushing out grafting, any grafting on such oily skinned tips leads to almost no effect is done in a standard manner; all gestures in this cases have to be massively over corrected given 75% of its effect is masked by the skin (opposite to thin skinned cases).

Seek the advice of an experienced revision rhinoplasty who shares this technical philosophy.

Since we can see this well circumscribed circular piece of cartilage through your tip skin, I believe your nasal tip skin is a bit thin, not thick. We have had good success camouflaging nasal tip irregularities with microdroplet Silikon-1000, however, you also appear to have nasal tip asymmetry and a tilted columella. In that regard, it is not clear whether or not a permanent non-surgical approach would be feasible for you. If you have severe dissatisfaction with the appearance of your nose right now, you should bring up these concerns with your surgeon, and consider getting an opinion from another reputable rhinoplasty specialist.
