when your surgeon performs gynecomastia surgery they usually leave a small portion of the gland behind the nipple to prevent the caved in or crater look that can happen when the nipple loses support or excessive excision is done and the skin sticks to the underlying muscle. Another possibility is recurrence that is if there a cause for the gynecomastia ( hormonal abnormality) that hasn't been addressed it can recur as some of the gland is left in place and that might grow in size if hormonal stimulation is still existent though given the time frame it's more likely retained gland from the original surgery. If this continues to bother you down the line , a revision surgery can be done though removal of more tissue will carry the risk of potential deformities.I hope this helps , best of luck!

Protruding nipples can be due to several causes: loose skin that hasn't yet fully retracted, hard scar tissue under the areola, or even remnants of glandular tissue if it wasn't completely removed. The fact that the area appears flatter when raising the arms suggests mild sagging or maturing scar tissue. Additionally, a hunched posture can make the chest appear more prominent.
