Bulbous nasal tip and thick skin of tip: if a person has a bulbous tip but it is mainly due to the cartilage rather than the skin, the tip is refined by trimming the Cartlidge and doing various other things to the Cartlidge such as suturing. If the tip is bulbous primarily because the skin is excessively thick, trimming the Cartlidge will not have a dramatic affect. Good Afternoon! Thick skin in rhinoplasty can be challenging. In order to make improvements such as the photo, structure in the form of cartilage is needed to "stretch" the skin. Thinning the skin during surgery can be performed, though it should be done carefully in order to not damage the blood flow. Projecting - or increasing the distance the tip is away from the face - is a powerful maneuver to create narrowness on the front view. In short, there's lots of options!

A closed rhinoplasty approach can accomplish improvement and refinement of your nasal tip, however thick-skin cannot be changed and will be a limitation as to how much refinement you can obtain in the nasal tip itself. When patients have thick skin, they also require multiple steroid shots in the first several months after the surgery to reduce excess swelling present. The dorsal hump can also be shaved down, and the bridge line narrowed with all the incisions placed on the inside of the nose. No external incisions are required, no painful packing is required either. Digital computer imaging on your nose upon your facial features would be helpful to understand what can and cannot be accomplished

Your photographs depict an individual who apparently demonstrated a broad nasal base and a relatively flat nasal bridge and a second photo demonstrates an individual with an attractive normal nose.
Rhinoplasty can correct a broad nasal base and a flattened nasal bridge. It unfortunately cannot correct thickened skin. However, most patients with thick skin who undergo rhinoplasty appreciate their improvement in appearance although the result is not as profound as in individuals with relatively thinner skin.
