Will another round of Kybella fix this? (Photo)

Question

I’ve had 2 rounds of kybella over a year ago, along with additional weight loss over the past year. I am left with this. I am able to squeeze it like there is fat there but I’m nervous skin laxity will be worse if I do another round. However a neck lift is not in my budget. Is it worth trying one more round of kybella to get rid of what’s left??

Hi NicoleYou will probably benefit from skin tightening at this point. Ultherapy or RF microneedling are good options.

Dear Thatgirl_MA,
Kybella typically takes 3 to 5 treatments to work. This is why I've stopped carrying Kybella in my office. I recommend neck liposuction more often then any injectable procedure for the neck now. It provides superior results gives me more control over the area than any injectable. It also costs about the same as what you paid for your first kybella session. The liposuction can be done under local while awake in the office and recovery is about the same as kybella.

Keep in mind, Kybella is good for dissolving the submental fat under the chin area while contouring the jawline and making the chin more defined, but a combination approach is needed to get the most improvement with non-invasive options. I recommend a combination approach with Thermage, Ultherapy, and Exilis.

Your side profile photograph demonstrates a recessive chin profile for which a chin implant can improve. You also have extensive fat deposits located both above and below the platysma muscle in your neck. This is going to require a neck lift procedure. This involves liposuction of the fat deposit above the muscle and surgical extraction of the significant fat deposits  below the muscle which also includes a platysma plasty to significantly improve the jawline.

Your pictures are insufficient to make a good assessment.
To make an assessment we need quality pictures taken at proper angles which must include a neutral profile picture.
A great majority of patients seeking fat removal in the chin and neck area whether by chemical fat destruction or liposuction have it as a primary underlying problem a facial skeletal imbalance with a mandible that is small or lacking in projection in comparison to their maxilla.
The primary cause for a premature double chin is usually not due to an abnormal fat distribution but instead related to bone structure and how that causes soft tissues to drape in an undesirable manner.
I suggest having a few in person consultations for local board-certified plastic surgeons to get a quality assessment and better understanding of treatment options.
This anatomic area and the dynamics of what causes it to look certain ways is sometimes misunderstood and it’s not uncommon for different providers to have different recommendations.
It is not an easy or straightforward area to either make assessments for do consistent quality treatments.
There are a significant number of tissue layers that can all contribute to how the area appears. These include bone structure, skin, subcutaneous fat, but is my muscle, submit this more fat, submandibular salivary glands etc.
Fat removal works best for individuals who have thicker layers of fat and in particular young people who have excellent skin elasticities.
