Am I a candidate for thigh lift revision to have them smaller and smoother? (Photos)

Given your situation, I would recommend a two-stage surgical approach. The first stage involves circumferential 360-degree thigh liposuction, addressing the inner, outer, front, and back of your thighs. This procedure will not only refine the thigh contours but also allow for fat harvesting, which could potentially be used for a Brazilian Butt Lift (BBL) if you desire enhancement in that area.

Approximately 4 to 6 months following the liposuction, a second surgery could be considered to revise the thigh lift. This would utilize the existing vertical scars from your previous surgery, running from the groin to the knee, to remove any residual loose skin and achieve a smoother appearance.

This staged approach allows for a comprehensive assessment and correction, potentially fulfilling your goals for smoother, smaller thighs.

Thigh lifts can be very challenging.  Often the result of weight changes or previous surgeries, the thigh skin can sag.  The weight of the skin on the suture line can also be significant.  The further the area of concern is from your incision line, the less dramatic of a lift you will see.  You would not want the scar to migrate south, and become more visible because of increased tension.  Discuss with your doctor what changes you hope to see.  Often, the texture of stretched thigh skin and contour irregularities cannot be improved by putting more tension on the skin, and other techniques may have to be considered. Communication is your best option and I agree that discussing this with your original surgeon is the best way to start.
