Congratulations on your weight loss! When you have completed your weight loss journey and are at a stable weight you will likely benefit from the fleur de lis incision pattern. This will remove the most excess skin and give you the best overall contour. I've included a video I hope you find helpful.
You will most likely need a circumferential lift with the massive weight loss of 92 lbs. This technique avoids the need for the fleur de lis. I would not recommend a standard tummy tuck in your case.Best Wishes,Gary Horndeski, M.D.
For my patients that have lost close to 100 lbs, they often have significant hanging skin in a horizontal and a vertical fashion. So removing the skin in both of those directions with a Fleur de Lis abdominoplasty would be best. However it comes with a vertical and a horizontal scar. Most of my patients are so relieved to get rid of the excess hanging tissue and don't mind the scar. We often use Embrace scar management system to help with the scarring.
Congratulations on your weight loss and lifestyle change! If the submitted photos are current- then I would consider additional significant weight loss. The vertical component is reserved for reducing the circumferential excess. The horizontal excess that is the tissue hanging below your belly button is treated with extended transverse incision. If you proceed with a tummy tuck now- before any further weight loss, the low transverse incision should be adequate

I would suggest speaking with a board certified plastic surgeon, but many times after weight loss a fleur de lis is used to accomplish the goals my weight loss patients are looking for, but I review each case individually and based on the patient's goals. I applaud your efforts and patience as you work towards your goals. You will likely be an excellent candidate for standard or extended tummy tuck surgery at some point. Achieving this long-term stable weight, will increase the safety of the operation (help avoid serious problems such as deep venous thrombosis and pulmonary embolism), will likely improve the outcome of the operation, and will decrease chances that additional surgery will become necessary subsequently.In my practice, I do not ask specific patients to achieve a specific weight prior to proceeding with tummy tuck surgery. Again, I simply ask patients to achieve a long-term stable weight where he/she feels comfortable and does not expect significant fluctuation postoperatively. Personally, I do not find the use of specific numbers or BMI values applicable in all cases. In my opinion, nothing replaces in-person evaluation and individualized assessment and recommendations (based on the priority of SAFETY). When the time is right, exactly what operation you should undergo will depend on your physical examination and personal goals. Most patients are able to undergo a standard or extended tummy tuck incision using a transverse incision. Occasionally patients who have significant tissue laxity (vertically and horizontally) benefit from the fleur-de-lis tummy tuck procedure. Make sure that you are working with a well experienced board-certified plastic surgeon (who can demonstrate loss of experience achieving the types of outcomes you would be pleased with), well experienced anesthesia provider, in a fully accredited surgery facility.
