I think the problem was exacerbated by significant weight loss, but that happened quite a long time ago. Would fat transfer be a good solution to this? I'm concerned about the impermanence / long term impact of filler. Would orthatropic treatments to expand the maxilla be effective? Thanks!!

Answer: Best solution to nasolabial folds and hollow cheeks? Thank you for your question and photos. It is a little hard to say without at physical exam. It looks like you are a bit deficient in the cheek area. You have a bit of ta nasojugular groove. Your are also a bit deep in the NLF. This can occur for a few reasons. Your malar fat pad looks like it has become a bit smaller and has moved downward. Sometimes augmentation of the malar pat pad area can help. There are many different materials that can be used. Filler is usually the first choice. Directly injecting the malar fat pad area and the NLF area is the most common approach. Again, hard to day, but I think you could augment the soft tissue with good result, as opposed to moving the bones of your face. Please visit an experienced, well trained injector.
