Left upper eyelid margin thickening 4 years after double eyelid surgery. No one can explain it About 4 years ago, I noticed the inner upper eyelid margin (right along the lash line) of my left eye had become swollen and thicker. There is a clear, visible difference compared to my right eye. This makes my left eye look noticeably smaller, less of the white of the eye is visible than before. I originally thought it might be a blocked gland, so I've had it manually expressed by a specialist multiple times, and I've also been seen at several hospitals to check for infection or inflammation. Every time, I'm told there's nothing wrong. I had double eyelid surgery years before this started, specifically to make my eyes look bigger and now one eye looks smaller instead. I don't understand how this can happen, and I'm worried it will keep getting thicker over time. Can anyone help explain what might be causing this, and is there any solution or treatment that could help?

A thickened upper eyelid margin years after double eyelid surgery can come from several causes, and it is not always related to the old surgery. Possible causes include chronic meibomian gland blockage, blepharitis, chalazion-type inflammation, scar tissue, eyelid malposition, conjunctival thickening, or a small benign eyelid lesion.
If multiple exams have shown no active infection, the next step should be a very focused evaluation by an oculoplastic surgeon, not only a general eye exam. They can assess the eyelid margin, tarsal plate, meibomian glands, lash line, old surgical scar, eyelid height, and whether there is subtle ptosis or asymmetry making the eye look smaller.
The fact that the eye appears smaller may not be only from swelling. It could also be related to eyelid position, scar tethering, levator function, or asymmetry in how the upper lid opens. These are different problems and need different treatments.
Treatment depends on the diagnosis. If it is gland-related, warm compresses, lid hygiene, anti-inflammatory drops, oral medication, or in-office gland treatments may help. If it is scar tissue or a structural eyelid issue, revision eyelid surgery may be considered. If there is a localized thickened area that keeps growing, biopsy may be needed to rule out a lesion.
My advice would be to take old photos, current photos, and your prior double-eyelid operative details if available to an oculoplastic specialist. Ask specifically whether this is eyelid margin disease, scar-related thickening, ptosis, or a small lesion. It may be fixable, but the correct treatment depends on identifying the exact cause first.
