I am 5'6", 115lbs, 32yo, breast fed 2 babies, and starting at a deflated 30A. I am planning on getting 500cc UTM silicone HP implants. I do not need a lift, but am worried about nipples being too low as a result of too much "upper boob" above the nip and not enough lower pole to elevate it up.With HP, will that be an issue, or should I expect my nips to rise to an elevated position once fully healed?I have seen some HP results have little lower pole and unsure what causes it?
Answer: Nipple to fold distance on stretch For a 500 cc implant, the distance of your nipple to fold when pulling nipple/breast tissue toward clavicles should be 10-11 cm to have a pleasing nipple position when drop and fluff has occurred, about three to six months.

Answer: Nipple position Dear KayPg, breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small.You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals. Answer: Implant selection The implants you have selected seem like they may be large for your frame. Implants do not tend to raise nipple position much. They can create more upper pole fullness and obviously add volume. I've included a video on implant selection that I hope you find helpful.

Answer: Surgery The nipples won't ride up much. The implant size you are choosing is too large for your frame and tissue. I would suggest going with something that is smaller and more comfortable on your neck and shoulders.

Answer: Small Chest and Large Implant Your concern is quite valid. Typically, inserting such a large implant into a smaller breast pocket increases the risk of a less natural look. While it's not to say it won't suit you well, the current photos don't support. Opting for a slightly smaller implant could tilt the odds in favor of a more natural appearance while still achieving fuller, beautiful breasts. Dr. B.
