Is a sliding genioplasty a good option for me? (Photo)

I have always hated my side profile and I I always have excess fat under that area no matter my weight (currently at a normal weight). I have braces for 5 years to fix an overbite and they had mentioned jaw surgery but backed off that.

You are a good candidate for a genioplasty. You have what we call a retrusive chin. This procedure would bring your chin forward, to better balance out your side profile. It would also elongate and add definition to your jawline, and tighten the skin underneath. You may also look into jawline contouring for added definition as well! Another idea could also be buccal fat removal. This would reduce some of the fullness in your cheeks and the jowling around your jawline.

Your fat distribution is normal and no different in the lower third of your face and neck than anywhere else. The primary problem is related to your facial skeletal structure. In particular your mandible is small in comparison to your maxilla.
The best treatment options come from addressing the primary problem head on. Soft tissue manipulation will only give partial improvements at best. recognize that when you go looking for treatment options. Not everybody wants double jaw surgery.
Liposuction isn’t going to magically make your mandible bigger.
To get a more skeletal oriented assessment consider consulting with oral surgeons but ideally I recommend you have a few consultations with the oral surgeons and a few consultations with local plastic surgeons.
There is an overlap and both specialties contribute to how to treat this part of the face.
Simplistic approaches will probably not give you great results.
