It appears as if you had both breast augmentation and a breast lift. The skin "bunching" you are referring to occurs from the "lift" portion of your procedure. The extra breast skin is gathered and can look raised or bumpy. This will smooth out and flatten over time. Relax and take a deep breathe; you are very early on in the postoperative recovery period. It will improve dramatically over the next several months.
Yes- The appearance of a scar one week from surgery can be bunchy as the breast swells. It takes about 3 months for a scar to settle. Discuss your concerns with your surgeon earlier rather than later. This will make you feel better sooner and will alert your surgeon to your concerns. Follow their post operative scar care instructions as well. It really does help the scar look better in the long run.

Some puckering around incision lines is due to internal sutures which often dissolve around 6 weeks post surgery leaving a much smoother appearance. Additional smoothness is obtained when the implant or breast tissue settles into the lower pole, which may take 3-4 months. "Dog ears" or standing cones at the very end of incisions result when tissue is tightened, leaving relatively lax tissues at the ends of incisions. While the appearance of these often significantly improves with settling and reduction of swelling, there can be a small residual fullness. Be patient, you will see improvements week to week. HI and thanks for posting your question and photos!It appears that you had a breast augmentation/mastopexy procedure which is designed to both increase the volume of the breast and retailor the skin to allow for ideal positioning of the nipple and areola on the breast mound. The incisions are planned and made to account for future stretching of the skin and healing of the incision without allowing too much tension. The timing I usually discuss with my patients to allow for these areas to settle out is 3-4 months. It even takes more time after that to allow for incision scars to settle and fade (6-8 months.) You are very early in the course and things will likely settle out nicely!Best Wishes!

Puckering and bunching of the wound at one week is likely to improve significantly over the next few weeks. However, one could not be confident that it will resolve completely. If after six months the scars have matured and the puckering persists, then a further small, local anaesthetic procedure may be helpful.
