The key to long-term improvement is a combination approach. I recommend fillers to soften the nasolabial folds themselves and also lift around the mouth and cheeks to build underlying anatomical support. Everyone should have a nasolabial fold especially when they're smiling like you do in your photograph. I would like to see your photograph with a face being relaxed. you also need to provide more of the face visible, at least from the eyes down as to determine the cause of your nasal labial folds. What a great question! Subscision is typically an option for deep creases in this area. First we should establish whether you have folds or creases. Folds are described as ‘hills and valleys’ or contours in the area. Creases are etched lines in the skin. Subscision works well for creases and releasing the ‘tacked down’ skin. I will often subscise superficially while laying filler into the deeper folds and a more superficial layer. The other question is whether you have loss of volume at the cheek, or lateral face that is adding weight to the laugh lines. Lastly, for hyper-dynamic muscles, you may want to find a very experienced provider who would be willing to try small amounts of toxin to limit the strength of the muscle contraction. If injected properly (right amount in the right place) the result could help the overall appearance of the laugh lines. Hope this helps.
