Our office has many different treatments for acne scars depending on the depth of atrophy or hypertrophy. Fractional Thulium laser and clear & brilliant can also reduce the inflammation and redness of the scar when combined with topical therapy. Fractional lasers such as Fraxel, erbium, and CO2 laser can be combined with radio frequency and also fractional RF to improve textual issues and microtopography. Subcision and TCA cross are also used for deep ice pick scars and narrow valley scars in order to improve the micro texture. Keep it simple and relatively inexpensive. The simplest solutions are often the best solutions. The use of subcision, which I have been using for rolling and boxcar scars since 1995, has a proven track record for both safety and efficacy for elevating and smoothing rolling and boxcar acne scars. Following this, professionally performed manual Dermastamp microneedling (no need for the added expense and risks of RF), another simple and relatively inexpensive technique that I have been using for nearly a quarter of century can then be used to superficially blend the texture and color of the treated areas better with the surrounding areas of normal skin.
