Asymmetrical, sitting low and far apart nipples - What procedure would I need to correct the shape of my breasts?

determining whether you need simply a breast augmentation versus a breast lift can be somewhat complicated. It depends on a number of factors including skin laxity and current nipple position. Generally speaking, if the nipples are lower than the inframammary line the patient will most likely need a breast lift. If the nipples are at or above the inframammary line we can generally get by with just a breast augmentation depending on the size of the implant the patient wants. I often still using implant when I perform a breast lift because it provides more upper breast fullness after the lift. To be sure a consultation with a board-certified plastic surgeon will help determine what the patient actually needs.

The technique I recommend is The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. Later, fat transfers or small implants can be placed if additional volume is desired.

Thank you for your question. After losing that much weight, it is normal to see loss of breast tissue, giving a ptotic, or sagging look. However, based on the photos, the nipples appear close to the level of the inframammary fold, in which case augmentation with implants may help re-shape the breasts. However, augmentation alone may worsen the width of the nipples without additional works. A consultation with a board-certified plastic surgeon will help assess your needs and optimize your results. The good news is, this situation is correctable. In your case, the most appropriate surgical approach would be a breast augmentation combined with a breast lift. That’s because there is significant sagging and asymmetry in both your breast tissue and nipple position.
When augmentation is performed together with a lift, I believe your nipples will end up being more symmetrical and better aligned — resulting in a more balanced and aesthetically pleasing outcome.
However, if only implants are placed without performing a lift, I do not believe the result will be optimal in your specific case. The underlying tissue and nipple sag would not be properly addressed, which may lead to a less satisfactory appearance.
In summary, if your weight loss has stabilized and you've reached your target weight, I would recommend a combined breast augmentation and lift as the most suitable surgical option for you.

From the photos in order to improve the asymmetry and shape concerns you will need to consider a lift in addition to implants. The result would be a perkier, uplifted look with improved symmetry.
