In my professional opinion, a breast lift alone — without implants — would be the most suitable option for you, especially if we preserve all your natural breast tissue. Even if you were considering an implant, the distance between your nipple and the sternal notch is quite long, which would increase the likelihood of revision surgery if augmentation is done now. With a well-performed breast lift, you would likely achieve a B, or even a small C cup, which might already be satisfactory for you. Now, if — after 6 months — you still feel that your breast size is not large enough, we could then consider placing an implant in a second-stage surgery. This two-step approach often leads to better outcomes. We often see in post-weight-loss patients that if we place an implant at the same time as the lift, the tissues (being very loose) tend to sag over the implant, leading to what's known as a "waterfall deformity". This usually requires revision surgery. That’s why I recommend first doing a breast lift alone, which would already restore shape and volume using your existing tissue. If you are still seeking more volume after 6 months, we could then safely add an implant in a second procedure to achieve the most optimal and natural-looking result.

However, if only implants are placed without performing a lift, I do not believe the result will be optimal in your specific case. The underlying tissue and nipple sag would not be properly addressed, which may lead to a less satisfactory appearance. Regarding the procedures you mentioned — liposuction of the flanks and thighs, BBL (Brazilian Butt Lift), fat transfer to the breasts, and nanofat injection to the face — these combined procedures are not expected to take 12 hours. One of the main concerns is fat embolism, particularly when fat is injected into the muscle layer. We now know that this risk is significantly reduced when the injection is done in the subcutaneous layer (between the skin and muscle), especially when using ultrasound guidance. So, if you are considering a BBL, I strongly recommend that it be performed as a "safe BBL" under ultrasound guidance, with fat injected only in the appropriate subcutaneous plane to minimize risks. Regarding fat transfer to the breasts, it’s important to note that the results are less predictable. While it can be useful for correcting minor asymmetries, it’s not ideal as a primary method for breast enlargement, because around 30–40% of the transferred fat may be reabsorbed within six months, which can lead to uneven or unsatisfactory results. However, based on your photos, I believe that even with exercise, it may not be possible to completely eliminate the loose skin in your abdomen. What you can do now is continue with diet and exercise until you reach your ideal weight, and then we can reassess the situation. From a plastic surgery perspective, I would say you are already a suitable candidate for liposuction combined with a tummy tuck, even at your current age. The excess in your lower abdomen appears to be due mostly to loose skin, which cannot be effectively addressed with liposuction alone. The only way to remove that excess skin is through abdominoplasty (tummy tuck) surgery.
