Perioral liposuction would usually not be my first choice for this concern. The area around the mouth has very small fat compartments, important smile muscles, and thin skin, so liposuction there can create dents, irregularity, stiffness, or an unnatural smile if too much is removed. In your photos, the line near the mouth corner looks like a normal dynamic fold from smiling and cheek movement rather than a clear pocket of fat that needs removal. If the concern is that the corners look closed or covered when you smile, the better starting point is an in-person assessment of your smile animation, lip support, dental show, and whether the corner is being pulled down by the depressor anguli oris muscle. Depending on the exam, options might include no treatment, very conservative filler support around the oral commissure/marionette area, or a small amount of Botox to the DAO muscle if the corners pull downward. A corner lip lift is a surgical option only for selected cases with a fixed downturned corner. I would be very cautious about liposuction close to the mouth for this specific goal.
