Festoons are not just overhanging hammocks of skin at the junction of the eyelid and cheek, but they may have fluid accumulation too. Festoons are different from eye bags. A lower blepharoplasty, which treats eye bags, may not be the right procedure for this issue, or it must be combined with other treatment modalities. I recommend scheduling an in-person consultation with a reputable board-certified facial plastic surgeon or an ophthalmic and oculoplastic surgeon who can properly diagnose and assess your skin concern, and then determine the best course of action to achieve your desired results.
Festoon treatment is a difficult one. There are several options: extended blepharoplasty with cheek lift, CO2 laser resurfacing, sclerosing agents, face tight, and direct excision. You need an in person consultation to discuss the pros and cons of each and which is best for you.

Thank you for your question. I think that you have festoons, or malar mounds, or malar bags, and this is a significant and highly distressing problem. The treatment of festoons is quite complex. I believe that treating this condition is solely in the purview of an oculofacial plastic surgeon well versed and experienced in treating this condition. Avoid general plastic and facial plastic surgeons, the best of whom would or should decline to treat you, in my opinion, as they simply do not have the necessary background knowledge in the anatomy and in the pathophysiology most likely causing the problem. Without doubt, I would avoid further fillers to your cheeks and tear troughs and would avoid fat transfer too (even though this was recommended by a general plastic surgeon who also answered your question). First, you should dissolve, perhaps several times, any fillers you have had, no matter how long ago you had them, with hyaluronidase. After this, you could consider radiofrequency microneedling or ablative laser treatments. Personally, I have had much success treating this with surgery (have a look at the real patients, many of whom travel to me from other states, I have treated on my website), but as with any surgery, there is never a guarantee. What you are describing as festoons or malar bags are an area between the lower lids and cheek that are susceptible to fluid retention. The best solution that I have seen in my practice is an extended lower lid blepharoplasty with SOOF lift (suborbicularis oculi fat pad). This helps to address the static portion of the aesthetic problem. However, periodic fluid retention in this area can still occur, but to a lesser degree.
