Hi I'm 46 and I'm noticing my face is sagging really bad, I've lost over 30lb and did have a fuller face not so long ago, my nose looks crooked and large and bulbous at the end, my philtrum looks wide and longer it looks very masculine now, my top lip has thinned out, my cheeks are flat and sagging, nasolabial folds have deepened now have marionette lines developing, my chin looks longer and jaw looks irregular shape, look like I have Acromegaly, as nose,chin,jaw look bigger.

It would be very difficult to talk about acromegaly based on this photo. What is paper er, however, are typical signs of early facial aging with howls, skin laxity and deep nLF and M lines. In person consultation with a board certified plastic surgeon with good reputation in facial rejuvenation would be a good starting point. Good luck.

Based on your photo, the soft tissue (skin and fat) changes that are present are most likely due to loss of fat and subsequent skin looseness in the face.  This resulted in flattening in the upper and mid cheeks, with downward sliding of skin and fat developing jowls, with the nasolabial and marionetted folds in front of the jowl bulges.  Basically facial fullness has decreased in the upper face and increased in the lower face, creating a squarish shape.  This could all be improved nicely with a standard facelift, possibly with either fat or filler placed in the upper cheeks to increase prominence there.  The flattening and elongation of the upper lip is also an aging change that can be improved by a lip lift.  As for your nose, a rhinoplasty to improve nasal shape, especially the tip, is possible but less predictable in older patient's.

No, you are just noticing changes in your face from losing weight. If you have more weight to lose, then do noninvasive micro-needling with radiofrequency treatments like Secret for now to help tighten your tissue.  You could also check into a rhinoplasty if you are concerned about your nose. Weight loss did little to change your nose.

In my opinion, if you get facelift, fat injection and liplift with appropriate techniques done in a session, your face will look younger and more feminine. Also to  Add rhinoplasty ,it will be perfect result.
