Deep spongy lump after BBL revision. Is this a lipoma or something else? (Photo)

Question

I had a BBL in December 2022 and due to a large fat pocket that formed on my left cheek I had to return and have revision in March 2023 (this year) which they used liposuction to remove the fatty lump. all seemed fine until 2 months ago when the liposuction area felt tender and warm on occasion to touch. There is slight bruising or discolouration as it doesn’t hurt in surfacing areas except in the surrounding areas when pressed it feels quite deep. Is this a lipoma or something else?

I understand your concern. However, without a proper assessment, it would be difficult to determine what is wrong. It is best that you visit your plastic surgeon for further assessment or ask for a second opinion. Only after a thorough examination, you can get proper recommendations and advice.

Very likely that this is fat necrosis; portion of transferred or detached fat tissue dies and melted into a liquid state. This needs to be drained and left to mature for few months before new correction. Hope this helps.

Despite your attempt at describing your condition, well and including pictures, we can’t really make an assessment based on the information included. To have a better understanding of what’s going on you really need to be examined. You could have a fluid collection called the seroma.
It’s unlikely you have a lipoma, unless you had a lipoma there before your first procedure.
The most likely explanation is a combination of fat necrosis from the previous fat grafting with a poor aesthetic outcome based on how the procedure was performed.
Delivering consistent quality, liposuction and fat transfer results is not as easy as people think. The number of plastic surgeons have truly mastered these procedures are in a small minority. If you’re provider cannot give you the care you need or a satisfactory explanation that I suggest scheduling a few in person second opinion consultations.
 For second opinion consultations I recommend patients come prepared bringing with them a complete set up before and after pictures and copies of your previous operative reports. These are part of your medical records that can be obtained by simply asking for them from your current provider.
