Hello, I am considering a breast lift and exchanging my 600 cc high profile silicone implants under the muscle for a much smaller implant, 300 to 325 cc silicone. I have absolutely no breast tissue. It’s just stretched skin so my concern is, will it be more difficult of a surgery to perform with deflated skin to get the desired result of a much smaller breast and is it feasible to go from 600 cc to 300 to 325 cc for a more natural look?

You probably tore part of your capsule and now the implant can shift a little more. It may not be a problem at all, but I would suggest going back to your doctor and being checked.
Considering the asymmetry you've shown in your preoperative photos, you have come a long ways following your first operation. In complex cases like this it is not unusual to require a revision. Many options may be available, but to select the best one would require an examination of your breasts...
Just removing the excess skin in the office can be done under local anesthesia, but this will be a temporary fix only. You actually need to repeat the lift and reposition your breast tissue and implant. That is not done under local anesthesia.
