Personally I think you look better without the paper towels.

I suggest having a few in person consultations for local boards certified plastic surgeons to see what some treatment options may be. You may want to consider a few rounds of temporary treatments with fillers before committing to something permanent to make sure you like the effect of having volume augmentation.

Facial fat transfer is less predictable and a bit unforgiving compared to fillers. Provider choice is key when it comes to getting consistent quality results.

It wouldn’t hurt to get an assessment from an oral surgeon or someone who does these type of procedures as well.

If the person who did your procedure is still practicing then following up with him or her would be your first stop in my opinion.

Some of your changes are due to simply getting older. It has been 17 years since your surgery after all. In your present condition you you have a nice ogee curve on your oblique profile, which is usually considered ideal, though you seem to want a more elongated, flatter curve.. However, you are losing fill also in the central face. Filling the central face and the lateral cheeks is a reasonable option. You may not be able to achieve your “paper towel” cheeks without multiple grafting sessions, however. Make sure you see a plastic surgeon with significant experience in fat grafting and facial rejuvenation.
