You may have a number of options available to you in order to lift your breasts as well as reduce the size of your areolas. You may also wish to have breast implants in order to increase fullness and improve shape. Be sure to book a consultation with a Board Certified Plastic Surgeon who is experienced with these procedures. All the best! 

Based on the look you are trying to achieve I would recommend a breast lift with an augmentation and an areola reduction. Please see a board certified plastic surgeon to discuss your options further.

Thank you for your picture and question.A lift will help to improve the "sagginess" as well as reduce the size of the areola. An implant will give you more volume and upper breast fullness.  Be sure to consult with a board eligible/certified plastic surgeon to go over all the details. At the consultation, you may find that 800cc is rather large. At 18 years old, if you get an 800cc implant, you definitely need to prepare for an additional surgery to include a secondary lift in the future.I hope this helps.- Dr. Bryson Richards, MD

I would recommend a lift, areolar reduction and modest implants.  This would improve the position of your breasts on your chest and deal with the asymmetry.  I think you would also like the results of adding a modest sized implant to increase volume and upper pole fullness.

You shouldn't even have to ask for an areolar reduction as part of your lift; it should be automatic. As far as implants are concerned, be careful asking for very large ones. They stretch and thin the tissues over time and can lead to recurrent sagging and long-term problems.Find someone skilled at doing breast augmentation with mastopexies.  They can be difficult because with breast augmentation, the skin is being stretched and with a mastopexy the skin is being reduced. Essentially, the two operations are working against each other. Because of this, many surgeons recommend staging them into two operations. I don't routinely recommend staing into two operations but I would with someone who insists on really large implants.

Thank you for your question and pictures.  I think that getting an areola reduction and lift is a great idea for you.  That would give you the perky breasts you desire.  It appears you have enough breast tissue, that an auto-augmentation mastopexy would give you that look while avoiding an implant.  800cc breast implants are just too large for you and I think it will cause problems down the line.Get a consult with a plastic surgeon to plan your surgery and give you all the information you need.

You have enoiugh breast tissue for an aug-augmentation mastopexy. This procedure lifts the breasts and also repositions the breast tissue so the breasts look perky and full without the need of a breast implant

Thanks for writing,I believe that when your breast are re-positioned, re-shaped and areolas are appropriately sized you may find that you actually have a lot of breast tissue. At age 18, 800cc implants will make you a breast cripple down the road. You will need further lifts / breast work, potential back, neck and shoulder pain, and don't forget about breast changes with pregnancies. Then there is the associated cost with each of the subsequent procedures. I would suggest to you start simple; get a great mastopexy first, after a period of time if the volume is not what you want you can always add implants.
