I’m 27 years old and have always struggled with genetically hollow undereyes. In 2016, I started my filler journey and got multiple rounds in 2018, 2019, 2021 and 2022. I also got PRP in 2019. I believe sometime between the 2019-2021 mark I began experiencing migration. I am in distraught of what to do as I have read horror stories about hyaluronidase. I notice these lines specifically when I smile. I notice ”fullness” / lumpiness looking upwards. I’m unsure what kind of doctor to reach out to, how difficult my case is, what are options and risks associated.

It sounds like you may have filler migration in the tear trough area this is common, especially after several rounds over the years. Filler (usually hyaluronic acid–based) can slowly move from the original placement, creating puffiness, uneven texture, or visible lines when you smile or look up.
Hyaluronidase, when done properly, is safe but because the under-eye is delicate, it’s essential to have the procedure done by a board-certified dermatologist, oculoplastic surgeon, or facial plastic surgeon experienced with dissolving filler in that area.
Possible (though uncommon) side effects include:

Temporary swelling or bruising

Minor tenderness or redness

Rare allergic reaction (skin test can be done first)

Temporary hollowness until filler or volume naturally settles or is replaced later
