Breast implant removal after replacement and lift - what is the best option for a good visual outcome? (Photos)

I am a very healthy 58 year woman. I had my first set of breast implants replaced with a breast lift in 2018. I now have capsular contracture and want to have them permanently removed. I am currently a c-cup and was originally a lg A cup with uneven breast size before my first set of implants at 20 years old. I am trying to decide what my options are having the best outcome for symmetry and look. Since I already had a lift, would I have better visual outcomes from another lift or fat transfer, or something I am unaware of? I may have enough fat to transfer? I am less concerned with size and focused on the final visual outcome.

I recommend explantation and lift using the Bellesoma. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. The weight of the breast is transferred to the underlying pectoralis major muscle resulting in long term stability. Vertical scars are avoided and nipple sensation (in 95%) is maintained if not injured during your previous surgery. Later, fat transfers can be performed if additional volume is desired.

Thank you for your question.  I have been in practice for over 30 years and revisionary breast surgery is a large portion of my practice.  Although many patients just want implants removed and a lift, their anatomy does not support that.  Most of your breast volume is related to your implant.  Fat transplantation to the breast does not work very well as the recipient site of the breast has very little blood supply, and the fat rarely gives any volume at all.  That is a big mistake for many people thinking that fat will given the volume they want.  I believe you would be an excellent candidate for reductive augmentation.  It is a procedure that I developed/invented in which I reposition the implant in a much higher location, reperform the lift, and remove inferior pole breast tissue.  You should be able to have a beautiful result.

Thank you for your question and photos. Clearly an examination in person would be required to specifically recommend the optimal treatment plan, however based on your history and photos it seems you would benefit from capsulectomies, implant removal, and mastopexy with or without simultaneous fat grafting, spending in the ultimate desired size. Your scar quality is excellent thus far, but your areolae are somewhat widened, and your existing mild ptosis suggests that a mastopexy would be of benefit. Do your research, be confident that your plastic surgeon understands your goals and shares your aesthetics. Best of luck to you.
