Hello I have some old migrated lip filler (3 years old) and I developed a white bump that is swollen along with the pooling of the lip filler. What can I do to correct this? Been a little depressed over it. Thank you

I can understand why this would be upsetting, especially when it affects such a noticeable area of the face. The good news is that in many cases, there are treatment options available once the cause is identified.

Based on your description and photos, a few possibilities come to mind. If you truly have residual migrated hyaluronic acid filler, it can sometimes create localized swelling, uneven contours, or small nodules even years after the original injection. Although most filler gradually breaks down, some patients do experience persistent pockets of filler that remain longer than expected.

The white bump itself may not necessarily be the filler. It could represent:

A small filler nodule or collection of migrated filler
A blocked oil gland or milia cyst
Localized inflammation around residual filler
Less commonly, a granuloma (an inflammatory reaction to filler)

The first step is determining exactly what the bump is. In many cases, high-frequency ultrasound can help distinguish retained filler from other types of lesions.

If residual hyaluronic acid filler is confirmed, hyaluronidase can often dissolve the unwanted filler and improve the contour irregularity. If the bump represents a cyst, milia, or inflammatory nodule, treatment would be different and may involve extraction, medication, or other targeted therapies.

One encouraging point is that this type of concern is usually treatable. While it may feel very noticeable to you right now, many filler-related irregularities can be significantly improved once the underlying cause is identified.

Try not to assume that the situation is permanent. Even when filler has been present for several years, there are often effective options available to restore a smoother and more natural appearance.

Three years should have long since resolved this to migrated filler forming a nodule instead of residual swelling. The white lump below your lip line is hyaluronic acid filler that moved past the vermillion border, became encapsulated by surrounding tissue and now shows through the thin skin just below the lip line as a pale lump. The pooled product sitting atop it is more of the product, only unencapsulated.

The solution for this issue is dissolving the filler with hyaluronidase, which I do in my office setting with the patient awake under a local anesthetic. One or two small injections of the enzyme, hyaluronidase, directly into the nodule as well as directly into the pooled product on top should break down and cause both to disappear within 15 minutes. Plan for this to be fully dissolved over two to three weeks as all swelling from the treatment resolves.

When this is fully dissolved, do not pursue more filler for a period of at least three months. Should you still want lip fullness to be added back to the area in three months' time, a surgical lip lift procedure and a small amount of hyaluronic acid filler, correctly inserted only into the body of the lip, will likely yield satisfactory results with an absence of the same complication occurring again, there are absolutely better options here than further hyaluronic acid injections.
