Hi. I've already had 3 open rhinoplasties and one closed revision. I've already had rib graft revision and the whole thing rebuilt. I've already had perichondrium and nano fat injected during the revision. I have been told I have extremely thin skin and also any more open revisions could compromise blood flow. I want more volume overall and I really want to fill in this dent. I don't want to do HA filler because I've seen Toriumi post that it's too risky. Is fat injection in the nose safer?

Hello. Thanks for your question. It could be from improper suturing and eversion of the skin. As well, grafting material may be needed to slightly push-out the columella to allow for a more smooth contour. There are options. Perhaps, seek more consultations to see what can be done.
This is an inverted V deformity. One of the most common errors I see in revision rhinoplasty when the initial surgery was performed by non-specialists. Revision (with a revision rhinoplasty expert) can resolve this for you.
