Question

5'4, 104 lbs, sientra textured round implant. My right breast seems to have the implant pushed too far towards the side. The breast is flat in the cleavage area and flat on top (compared to my left breast which looks round and plump). It seems that the implant has been pushed to the side vs. spreading out behind breast. Will this settle and shift? Or will this need a revision?

SInce you have textured implants, they should be adherent to the adjacent tissues, and therefore cannot be expected to shift or settle much at all. During healing your tissues will stretch, and swelling will diminish, so your own breasts will drop and soften up somewhat. But, your implants (if the texture is working as designed) should be pretty much where they were placed.You did not state whether or not your implants were submuscular or submammary. In either case, your own breast tissue seems to have NOT flattened out over your implant mound or submuscular mound, and the lateral "double bubble" is the result of this. (Nor would I have expected it to, since the texture of your implants adheres to the tissue where they were placed.)You appear to be very thin and with little of your own natural breast tissue, so I would have recommended submuscular smooth round implants as the best choice for your anatomy. Things will change over the next several months, but I think that revision may be necessary. I hope not, but hope is not a plan, so get as much information as you can and wait at least 3 months, though 6-12 may actually be better, if you can wait that long. We only see side  or oblique views so it is hard to tell, especially without preoperative photos. We also do not know if you have implants above or below the muscle.   I would suggest allowing enough time to pass to have the full healing take effect. That is a minimum of 3 months.  At each visit with your surgeon, he/she can see what progress you are making so keep those appointments up. Sometimes there are shaping maneuvers to help the implant pocket. A revision could be entertained if you are not satisfied when the healing process finishes. Discuss all options with your surgeon and hang in there.
