Unfortunately, any “fix” for your buttock dents will require surgery. If you are looking to just simply fill in the dents, fat grafting may suffice but may require more than one procedure. If you are also interested in adding a bit more perkiness and/or projection to your buttock then buttock implants may get the job accomplished in one surgery. Although many board certified plastic surgeons perform fat grafting to the buttocks, a relatively small number perform buttock implants and an extremely few number specialize in buttock implants. You probably would benefit best from butt augmentaiton with your own fat. If you are afraid of surgery or being put to sleep, we offer this procedure to patients awake in our office. Even a moderately small amount of fat could give you the improvement youre looking for.

These will need to be filled to even them out. The surest solution would be a fat transfer where fat is liposuctioned from one area, purified, and then injected into the butt and areas of depression.

it is hard to tell for sure without an examination. Based on your photo, it looks like you would get improvement after fat transfer. If you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Only after a thorough examination you will get more information and recommendations.

You can fix your buttock dents with fat transfer. High definition fat transfer, i.e. HD BBL, will provide you the best results.

The contour irregularity of your buttocks are not related to a volume deficit but rather a combination of how your skin drapes due to gravity and connective tissue stabilizing skin (same connective tissue that causes cellulite) that’s unique to the buttocks.

My best guess is that if you pull up on the skin on your lower back the area even salt very nicely and when you let the skin drop the contour irregularity will form again.

Simply adding volume may improve this but may also only partially improve it because the underlying problem is not directly related to a volume deficit. The problem is too mild to justify skin tightening surgery.

Likewise, liposuction of the fullness immediately adjacent to the indentation may give limited improvement.

There is no nonsurgical option that is going to fix this.

Adding volume with that transfer with selective Liposuction is probably your best option. This is probably going to help to some degree.

Expect different plastic surgeons to have different opinions and different levels of enthusiasm for correcting this.
