In my early 50s and never thought I'd be considering PS. I am fine with my face aging naturally, however, I have platysmal banding on my neck which I have become quite focused on and know will only get worse. I ran across this article with what appears to be a promising procedure. Any docs with experience performing this as an isolated procedure? Or what is the least invasive technique you would recommend in my situation?
Answer: Platysmaplasty combined with lifting Thanks for sharing a great question. In the younger patient lipo with a platysmaplasty is an option to treat bands when there is minimal redundant skin. Typically, patients in their 30-early40's may benefit from this isolated procedure. The procedure involves suturing the loose bands together show they are not so droopy. From the early 40's and on, the skin has enough laxity where a neck lift or lower facelift is needed in combination with the platysmaplasty. Combinging there procedures help to remove the lax skin. From what can be seen here, this would be the ideal treatment in my opinion. Consult with a few experienced facelift surgeons to discuss your options in person.

The best treatment would be to have the bands addressed and then to tighten the muscle from the sides as well. Then the skin can be trimmed appropriately for a smooth and rejuvenated neck. In order to perform a isolated platysma plasty, a one-inch incision underneath the chin needs to be performed and complete undermining of the neck skin to gain access to the muscle. This is not a closed procedure. The best option would be to perform a lower face and neck lift which addresses tightening loose facial and neck skin, tightening loose facial and neck muscles, and removal of any fatty deposits in the neck. Just a platysmaplasty from an under chin incision approach should provide decent improvement, but this would be less successful than a standard face and neck lift. The loose neck skin can fall back onto the underlying deeper tighten muscle after a platysmaplasty but there is a risk that it will sag a certain undetermined degree. So there is a trade off of less surgery for possible less successful tightening.
