It is difficulty to predict the exact change in size based on the amount of fat removed without knowing your height and weight and also without seeing if you have been using the correct bra size. It is usual to quote 100 t0 125 grams per cup size change for an average sized woman who is wearing a bra of proper size. At size 34, each 100 grams removed corresponds to 1 cup size change. 400 grams removed from each breast would take you down 4 cup sizes and would take you to an A, which is too small. Insurance companies usually require a minimum of 500 grams per breast so you may not qualify for insurance approval. The technique I recommend is The Bellesoma Method. This will reduce and reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. The weight of the breast is transferred to the underlying pectoralis major muscle resulting in pain relief without excessive reduction. If you currently have 800g of tissue per breast and 400g is removed they will be about half the size they are now. The band size around your rib cage will remain the same but the breast itself will be smaller and more lifted since a lift is part of the procedure. You can anticipate going down multiple cup sizes.

I perform many breast reductions and work with insurance companies often and it is always a difficult discussion with patients when it comes to insurance requirements for amount of tissue removed to obtain medical necessity coverage. It is difficult to say with any certainty what cup size you would be after a 400g removal bilaterally (especially not having evaluated you in person), but it is safe to say that you would be significantly smaller. I would encourage you to follow up with your board certified plastic surgeon to discuss your concerns in detail and to see if there is any possibility of conducting a peer to peer review if your surgeon thinks removing the insurance required amount of tissue might leave you with a disproportionate result. I think you need to think what you are really looking for. My guess is that if you remove about half of each breast, you will be left with roughly a B-cup. This will make you look a bit small, considering that you have a bit of excess weight in general. If you are looking to reduce the breast weight and are not as concerned about the size post op, then it would make sense to get the reduction. However, most insurance companies want you to remove 500gm per side, so be sure to get the insurance company answer in writing.
A 400 gram reduction is smaller than what the insurance carriers require in NY. It should be a reduction from an E cup, if that is even accurate, to a C. Whether that is too small is a decision that only you can make. You do have the option of a lift alone but then the cost would be entirely your responsibility.
