Cheek implant surgery can be done in different methods, depending on the surgeon. In most cases, I would say that cheek implants should not affect the sinuses. However, if an implant was secured with a screw that entered the maxillary sinus, then there might be a slight chance that the sinus could be affected. If the implant was secured externally for a few days, then the sinuses should not be affected.
Cheek implants sit on the outside of the bone that overlies the maxillary sinuses and have no connection to the sinuses which lie below and beneath it. Thus they do not cause any sinus problems. Uncommon cheek implant complications, like infection, can feel somewhat like a sinus infection but they can not cause an actual sinus problem.
Cheek implants are placed on the outside of the maxilla over the top of the bone covering the sinus cavities. In our practice, we do not screw them into the bone which could cause potential sinus problems. In our practice, we have not seen sinus problems with placement of cheek implants

Cheek implant inflammation and seroma after 2 months is probably what you have. I would drain the seroma and place u on antibiotics. If the drainage continued I would remove the implant--wait 2 months and put it back in. I would not use fat since it will not give you symmetry.
